Camp Registration Form

This form must be filled out a week before your camp date, if you have multiply children please fill out for each child. Lunch does NOT need to be filled out unless you are going to pay extra, lunch for the 4 days is $35 each child, it will be the same meal for the 4 days. Fridays we have FREE Pizza Hut personalize pizzas and popcorn for each child. If your child cannot have pizza and popcorn please send a bag lunch for your child on Friday. PLEASE NOTE... Anything that has a * by it MUST be filled out for it will not allow you to finish.

IMPORTANT INFORMATION:

  • Arrival: Camp drop off is between 8:30am - 9:00am. You will have to sign your child in daily.    

  • Departure: Pick up is at 4pm, your child will have to sign out daily.  

     $10 per day will be billed to your card for late pick up after 4:30pm.

WHAT TO BRING:

  • Water bottle with child's name on it

  • Snacks ( ALL snacks MUST be PEANUT & NUT FREE!!!) 

  • Hat, sunscreen and (bug repellent ~ optional) 

  • Clean comfortable running shoes required 

  • Painting Shirt (optional) 

  • Water clothes (NO bikinis) and towel. We will be visiting Muskoseepi park/splash park Monday ~ Thursday if weather permits.

  • Bring Lunch if you have not purchase the lunch option. 

CAMP INCLUDES:

  • Laser Tag Games Daily  

  • FREE T-Shirt 

  • Crafts/games (other than laser tag) daily

  • Outdoor activities Monday -Thursday (water activities as well) WEATHER PERMITTING!  

  • Movie, FREE personal size pizza and popcorn on Fridays 

  • Friday Funday, kids will get to play our 2 new mini games in the afternoon ( Check out our Time Freak/Lazer Frenzy videos on our "Video Page").

  • All afternoons during lunch will be a movie time, G or PG rated movies
